What is Continuing Care?

Continuing Care at St Peter & St James is for patients who are no longer able to support themselves at home, due to their diagnosis and higher dependency than services in the community can manage. This may be because of the complexity of their symptoms or the unpredictability of when they need support and care.

We have 14 Continuing Care rooms caring for people with many different conditions and prognosis.

Do I have to pay for this service?

Continuing Care is not a free service. Some patients do self-fund and the Clinical Services Manager will be able to provide information about this.

Some patients may be eligible for funding from the Clinical Commissioning Group and the Clinical Team will be able to support this application, if appropriate.

Referrals to this service can be in many ways – GP, District Nurses, Hospital, Hospice, family or self-referral.

The fees for care are dependent on the individual patient’s care needs.
