We work collaboratively with our neighbouring Hospices to ensure care is co-ordinated for the patient’s best interests. In areas close to the geographical boundary patients may express preference to come under our care, or it may be appropriate for referral to the service with which the GP surgery has established links in order to provide good continuity between primary care and specialist palliative care.
We care for patients from Burgess Hill, Haywards Heath, Lewes, Uckfield and the surrounding villages.
Our catchment area has a population of approximately 200,000 people. If every person in our catchment area gave just £13 a year this would cover our running costs.
